dd867b787f Allow parallel execution of distributed DDL 
Add distributed_ddl.pool_size to control maximum parallel to handle
distributed DDL.

Also:
- convert Exception constructors to fmt-like
- use sleepFor* over std::this_thread::sleep_for()
